Temporomandibular Disorder (TMD)


The temporomandibular joint (TMJ), or simply, the jaw joint, is the joint between the mandible (lower jaw) and the skull. It is located just in front of the ears and is the reason of a lot of people’s discomfort. The jaw joint is affected by several disorders that are collectively referred to as Temporomandibular Disorder (TMD). It is important to differentiate the symptoms of TMD from the harmless joint “clicking” or “popping” noises that more than one third of the population has. Here are some interesting facts about TMD:

  • Jaw popping or clicking noises in the absence of pain or other symptoms is NOT TMD.
  • Clicking or popping sounds are caused by structural changes of the joint and are usually harmless. More than half of clicking cases resolve with time although it may take several years.
  • Crepitation sounds, pain, limitation in mouth opening are signs of TMD. You should seek a consultation from your dentist.
  • All research studies failed to demonstrate any relationship between teeth and TMD! In other words, the way your teeth are or the way you bite have nothing to do with your TMJ and are unlikely to cause TMD.
  • As a result of the previous statement, TMD should NOT be treated by dental or orthodontic procedures. Although many orthodontists and dentists chose to believe that they can actually resolve TMD by doing some kind of dental or orthodontic procedures, there is not scientific evidence or whatsoever that supports these claims.
  • TMD cannot be treated, it can be only managed. Any TMD management should be simple and reversible like night guards (dental restorations, crowns, equilibration, surgery and braces do not fit these criteria).
  • Orthodontic treatment should not be started if you have symptoms of TMD. Orthodontic treatment will not make your TMD better or worse and should be delayed until your TMD is stabilized and under control.
  • The cause of clenching or bruxism (tooth grinding) is not well established but what we know is that they are NOT caused by malocclusions or dental problems. If you have a clenching or bruxim problem, the best management is using a night guard to protect your teeth from damage.
